Report date: Mar 24, 202538. Insects, rodents, & animals not present
- comments: observed evidence of appx. 30 or more mice droppings in kitchen-inside a open box of floor tiles on food storage shelf,on floor behind and in between refrigerator & stove. Mop sink closet - on the floor at entry door amongst clutter. Also evidence of 4- toxic soft packages of rodenticide (poison) on floor under mopsink and in rear storage closet along left side wall. Must remove droppings & poison clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 3/24/25. (priority foundation 7-38-020(a)(citation issued)

- comments: observed outter storm door door sweep bent along bottom and no screen on same to prevent insect entry.

- comments: must provide a splash guard or remove food items on shelving units next to hand sink in kitchen to prevent cross contamination.

- comments: must invert multi-use utesils on lower dish storage shelf.

- comments: must clean dust build up from cubbies and shelving units in class rooms. Must clean spillage and food dberis build up from refrigerator and bottom drawer of stove.

- comments: must repair or replace leaky middle pipe at 3-compartment sink.

- comments: must repair or replace torn/worn carpet in front of hand sink in class room,damaged & loose plank flooring to be in good repair throughout premises.
