2. City of chicago food service sanitation certificate
- comments: observed no certified city of chicago food service sanitation manager on site at start time of inspection, while eggs were cooked. The designated person in charge, who is physically on site, must obtain and maintain a valid city of chicago food service sanitation license. Violation corrected on site; the certified city of chicago food service sanitation manager, daniel snowden, arrived on site approximately halfway through inspection. Priority foundation violation #7-38-012. Citation issued

6. Proper eating, tasting, drinking, or tobacco use
- comments: found a food employee eating at prep table, drinking from a soda without a lid, and placing that cup on a prep table in the rear prep area. Instructed management that employees must consume beverages/food in designated areas to prevent contamination of food, equipment, utensils, etc.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed the following sinks not maintained with hot water: improper temperature of hot water observed at 1st floor toilet rooms at 72. 5f and 69. 8f. Instructed person in charge of hot water at all hand washing sinks must be at least 100�f. Priority violation #7-38-030(c). Citation issued.

- comments: observed next to 1st floor bar dish machine raw wood shelves, surfaces noted wood in said storage area to be water damaged and have mold, instructed to remove wood. To provide a smooth and easily cleanable surface

- comments: observed plastic used as liner on storage rack at bar, noted aluminum foil on prep equipment on 2nd floor. Instructed to remove. Noted rusted storage rack at bar, instructed surfaces must be smooth and easily cleanable. Instructed to remove or resurface and maintain

- comments: observed the interior of the reach in and walk in coolers in need of cleaning. Instructed to detail clean.

55. Physical facilities installed, maintained & clean
- comments: observed wooden floor in prep area warping where paint coatings have peeling off wood is exposed and under heavy equipment. I. E. Deep fryer wheels digging into wraping wood, instructed to repair to create a smooth and easily cleanable surface. Must maintain at all times.

58. Allergen training as required
- comments: observed the city of chicago food certified manager with no proof of allergen training certificate. Instructed all city of chicago certified food managers obtain this certificate.

60. Previous core violation corrected
- comments: noted continued non-compliance from report #2590487 conducted on 3/05/24. Previous core violations not corrected(55) 6-501. 12: found floors not clean behind and under heavy equipment and storage areas. Must clean. Must correct core violations by the date on the report. Priority foundation violation #7-42-090. Citation issued.
