- comments: observed no ambient air thermometer inside the hot holding unit. Instructed to provide and maintain.

38. Insects, rodents, & animals not present
- comments: observed approximately 15-20 mice droppings on site during inspection. Found the in the following areas on the floor: near the cooking equipment in the dihwashing/ food prep area, under the shelves, under the handwashing sink in the food prep area, on the floor under the dishware storage cages in the culinary room. Instructed to remove droppings, clean affected areas, and maintain. Priority foundation violation 7-38-020(a), citation issued.

55. Physical facilities installed, maintained & clean
- comments: observed a missing ceiling tile above the walkway in the serving area, near manager office. Ceiling observed water damaged and floor with water stains from a previous leak. Instructed to repair leak and replace ceiling time.
