- comments: observed low temperature dish machine not sanitizing food equipment and utensils, chemical ppm 0. Instructed manager to repair and maintain 100 ppm. Dish machine is tagged held for inspection and must not be used until reinspected by chicago department of health. Priority 7-38-025 citation issued

- comments: observed styrofoam cups used as dispensing utensils. Instructed manager to provide long handle utensils for dispensing food items and liquids to prevent contamination.

- comments: observed dust buildup on light shields, light fixtures, ceiling tiles, ceiling vents, electrical outlets, wiring in prep areas and ventilation fans in washrooms. Observed food debris on floor inside walk-in freezer. Instructed manager to clean and maintain.

- comments: observed no food handler training available for employees. Instructed manager to provide.

- comments: previous core violations not corrected from report #2560949 on 7-19-23:39-observed food items stored on floor in front beverage area, rear storage, walk-in cooler and freezer areas. Instructed manager to elevate all food items six inches off floor. 47-observed foil and cardboard used as liners on shelving units in beverage, prep, and storage areas. Instructed manager to remove. All food and non-food contact surfaces shall be smooth and easily cleanble. 53-observed no covered waste receptacle in women washroom. Instructed manager to provide. 55-observed missing grout on floor tiles in prep area and storage areas, damaged wall tiles in prep area, stained ceiling tiles in rear storage area. Instructed manager to repair/replace and maintain. Priority foundation 7-42-090 citation issued.
