- comments: observed no written employee health policy on site. Instructed facility to establish an appropriate employee health policy and maintain with verifiable signed copies on site for all food employees in acknowledgment of reporting agreement. Priority foundation violation #7-38-010. Citation issued.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed ice machine located in the rear food prep area with a build-up of black slime substance, dripping onto ice cubes stored at below ice bin. Ice is used for human consumption. Instructed to clean, sanitize and maintain. Priority foundation violation #7-38-005. Citation issued

16. Food-contact surfaces: cleaned & sanitized
- comments: observed ice scooper being stored on top of the ice machine with the food contact surface exposed. Instructed on how to store ice scoop between uses properly.

22. Proper cold holding temperatures
- comments: observed internal temperatures of the following time/temperature control for safety food items inside the two-door prep cooler to be improper: shredded chicken 51. 5f. Manager immediately and voluntarily disposed of 1 lb of product worth $11. 55 through denaturing process. Priority violation #7-38-005. Citation issued.

- comments: observed no labels for bulk black storage containers (rice, oatmeal, unknown seasonings) on the dry storage shelf near the office. Instructed person in charge of all bulk and working food ingredient containers must be labeled with the contents name.

- comments: observed torn door gasket inside of the hot holding door. Instructed to replace.

- comments: observed milk crates being used as shelving throughout the prep and storage areas and nehind sushi area in the far-right corner near the swinging door. Must remove and provide shelving 6' off the floor and provides floor accessibility to ensure adequate facility cleaning.

- comments: observed an accumulation of dirt/ dust debris buildup on the men's & women's toilet room exhaust ventilation. Must detail clean and maintain

- comments: observed expired food handler certificates for all employees on site. Instructed to maintain records at all times.

60. Previous core violation corrected
- comments: observed continued non-compliance from report #2568132 conducted on 12/07/22. Previous core violations not corrected: (#58)2-102. 13 found city of chicago food service manager without proof of allergen training on site at the time of inspection. Instructed to provide proof of allergen training for all city of chicago food service managers. Must correct core violations by the date on the report. Priority foundation violation #7-42-090. Citation issued.
