28. Toxic substances properly identified, stored, & used
- comments: noted an unmarked, unlabeled, open container of what appeared to be crystallized chemical sidewalk ice remover at the rear kitchen exit door. Instructed all working chemical containers that are not in the original container must be properly labeled. Priority foundation violation 7-38-015 citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: observed foods not protected from contamination. Upon entry to the food prep area observed a construction worker with a large electric chisel breaking and removing concrete floor tiles from the floor creating excessive amounts of dust and concrete debris in the air and splattering the area. Instructed to stop all construction at once. Noted an open container of griddle cakes contaminated by the dust and soot. Open beverage containers and food prep surfaces with dust and soot. Discarded all contaminated foods, instructed to wash, rinse and sanitize all contaminated utensils, food containers, food prep surfaces and equipment. Instructed not to resume any construction when business is open/operating. Priority foundation violation 7-38-005 citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: observed a large area of boxed and bagged dry foods and bagged to go cups and containers in the rear dry food area with splattered cement and dust on the surfaces directly next to a large area of flooring (approximately 3ft by 15ft) that was recently repaired with new, drying cement. Instructed to clean and sanitize all food containers before opening for use and discard all dirty cardboard boxes.

49. Non-food/food contact surfaces clean
- comments: noted dirty plastic ice shoots on both the ice machines at the soda dispensing area. Instructed to clean and maintain.

51. Plumbing installed; proper backflow devices
- comments: leak under the rear dishwashing area hand washing sink onto the floor. Instructed to repair and maintain.

55. Physical facilities installed, maintained & clean
- comments: missing/rearranged ceiling tiles in the rear dry food storage room due to construction. Instructed to replace.
