- comments: observed no valid city of chicago food sanitation certificate posted for pic on duty. Tcs foods ( eggs,etc being prepared,served). Priority foundation issued 7-38-012.
38. Insects, rodents, & animals not present
- comments: evidence of improper pest control. Observed 14 live gnats flying and 41 on walls throughout premises. Recommened pest control. Priority foundation issued 7-38-020(a).
55. Physical facilities installed, maintained & clean
- comments: observed floor need re-grouting in front drive thru area. Instructed to repair all areas.
