- comments: observed no disclosure and reminder on the printed menu for all animal food products that can be ordered raw or undercooked. Instructed to provide a disclosure on the menu for all items (eggs any style, steak, beef burgers etc. ) that can be ordered undercooked or raw and add a reminder on the menu that reads: *these items can be ordered undercooked or raw. Consuming undercooked or raw foods can increase your risk of foodborne illness. (refer to cdph consumer advisory handout) priority foundation violation #7-38-005.

- comments: backflow device at utiliy sink in rear prep area in disrepair and wrapped with plastic. Must repair and maintain.

Instructed manager to provide hand washing signs at all hand washing sinks used by employees. #37 observed bulk containers not labeled in front beverage and rear prep area. Instructed manager to label all bulk containers with common name of food. #47 observed damaged gasket on door of two door reach-in cooler in rear prep area. Instructed manager to repair and maintain. Instructed manager to replace and maintain. Observed towel used as stopper in 3-compartment sink. Instructed manager to provide adequate stoppers for 3-compartment sink. #55 observed dust buildup on wall vents in front prep area, food debris on floor in rear prep and storage areas.
