2. City of chicago food service sanitation certificate
- comments: observed no valid city of chicago foodservice manager certificate on site at the time of inspection. Instructed person in charge that valid original city of chicago foodservice sanitation manager certificate is required on site at all times food is being prepared or served. Priority foundation #7-38-012. Citation issued.

37. Food properly labeled; original container
- comments: observed bulk items in non original containers not labeled with common name. Instructed person in charge to label all bulk ingredients with common name and maintain.

38. Insects, rodents, & animals not present
- comments: observed approximately 100 mouse droppings scattweed throughout the facility in the following areas: behing and on the side of the oven, under shelving and equipment, on sides of storage bins, behind the cooler, inside the walk-in cooler under shelves, on the bottom shelf where equipment is stored near three compartment sink. Instructed the person in charge to remove all mouse droppings and sanitize all areas, and maintain. Priority foundation #7-38-020(a). Citation issued.

49. Non-food/food contact surfaces clean
- comments: observed build-up of food residue on bulk item containers, sides, and insides of coolers. Instructed the person in charge to clean and maintain

55. Physical facilities installed, maintained & clean
- comments: observed excessive unused items stored in the basement area on the floor. Instructed person in charge to remove and maintain all items at least six inches from the floor at all times.
