- comments: observed 40 rodent droppings on the lower surface of the deep fryers in the food prep area. Instructed to remove all droppings, detail clean, sanitize and maintain area. Recommended contacting pest control for assistance. Priority foundation 7-38-020(a) citation issued

- comments: observed employee in the food prep area not wearing a hair restaint. Instructed to wear a hair restraint when in the food prep area and when preparing/serving food. Maintain same.

- comments: observed the can opener in the rear with food and dirt build up. Instructed to clean and maintain can opener.

- comments: observed the chest freezer in the prep area with thick ice build up and food/dirt on the lower service. Instructed to remove ice build up, clean and maintain freezer.

- comments: observed the trash receptacle in the toilet room overflowing. Instructed to clean and maintain toilet room.

- comments: observed water stained ceiling tiles above the walk in cooler and in the food prep area. Instructed to replace ceiling tiles and maintain.

- comments: observed the floors in the corners, inside the walk in cooler, under/around the cooking/cooling equipments with dirt, food, grease and debris. Instructed to detail clean and maintain the floors in the rear and front throughout. Also must clean and maintain vents in the toilet room and in the front storage area.

- comments: observed the following core violations from report #2573554 dated 04/03/2023 not corrected and/or maintained. Instructed to correct/maintain the following: #10-observed the facility missing the required handwashing signage at the handwashing sink in the employee toilet room. Instructed to provide and maintain. #39-observed an open 15-25 lb bag of sugar stored in the cabinet at the self-service coffee station. Instructed to properly store said sugar to avoid contaminating the food product and maintain. #49-observed the microwave at the rear dry storage area with severe food debris build up and food splatters. Also, the insta pot (where the rice is cooked) stored on the shelving unit at the food prep area with food debris build up and not maintain. Instructed to clean and maintain said equipment. Priority violation 7-42-090 citation issued
