Report date: Jan 20, 202151. Plumbing installed; proper backflow devices
- comments: not able to locate a back flow prevention device at detergent/sanitizer dispenser unit behind three compartment sink where water and soap is dispensed. A black hose is attached from unit to water line of faucet. Must install so back flow prevention device may be located to be serviced and maintained. If unit is not in use remove said unit and hose.

- comments: observed expanding spray foam used as a sealant for openings around floor piping behind shelves throughout the premises. Must remove to be smooth and cleanable.

- comments: previous priority foundation violation not corrected from 1-6-2021, report# 2472343: #38) 6-501. 111 we observed evidence of pest activity in different places of premises. Mice droppings were observed in the kitchen: 30 droppings under and on top shelves,beside cooking equipment, 10 droppings at front window ledges. Rear storage area over 60 droppings under and behind cooler units. Instructed to clean,sanitize and maintain areas. Today 1-13-2021 still observed mice droppings throughout the premises: kitchen on top prep table by opened food, under shelves, beside ice machine and throughout storage areas. Instructed to remove all droppings, clean and sanitize equipment and surfaces. Have premises serviced by pest control company. Priority violation: 7-42-090, citation issued. Today 1-20-21 re-inspection requested by owner we still observed mice droppings in different location of premises. Over 50 rodent droppings. Also a live adult roach in the kitchen inside the microwave. Instructed to remove all droppings, clean and sanitize equipment and surfaces. Eliminate pest activity. Have premises serviced by pest control company.
