- comments: observed front and overhead doors not rodent proofed, 1/2" gap between front entrance door and over 1" gap on bottom corners of overhead doors. Instructed manager to seal all outer openings on doors to prevent pest entry.

- comments: observed leaking hot water faucet on 3-compartment sink in dishroom. Instructed manager to repair leaking faucet on 3-compartment sink and maintain.
54. Garbage & refuse properly disposed; facilities maintained
- comments: observed no waste receptacles at handwashing sinks. Instructed manager to provide waste receptacles in all areas waste is commonly discarded.
55. Physical facilities installed, maintained & clean
- comments: observed hand dryer not operable at rear handwashing sink near dishroom. Instructed manager to repair and maintain.
