2. City of chicago food service sanitation certificate
- comments: observed manager with an expired city of chicago food certificate on site at time of inspection. Instructed current person in charge that a manager with a valid city of chicago food certificate must be present during all hours of operation while tcs foods such as pizza, sausage, and meatballs are handled and prepared. Priority foundation violation, #7-38-012. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed missing handwashing sign at hand sink next to salad prep station. Instructed manager to provide handwashing sign indicating employees must wash hands.

36. Thermometers provided & accurate
- comments: observed no thermometer present in the 3 door reach in cooler in the pizza preparation area at time of inspection. Instructed to provide thermometer to monitor internal temperature of cooler in warmest area.

38. Insects, rodents, & animals not present
- comments: observed 10 rat droppings on the floor under shelves in corners of room in the side dry storage area where single service items, canned goods, and paper goods are stored. Observed 15 rat droppings along walls in basement water heater area. Instructed manager to clean, sanitize, and maintain area free of rat droppings, and recommended pest control service facility prior to reinspection. Priority foundation violation, 7-38-020(a). Citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: observed canned goods and boxed food items stored on the floor in the side dry storage location off main kitchen area. Instructed manager to elevate all items at least 6 inches off floor to allow for adequate cleaning.

41. Wiping cloths: properly used & stored
- comments: observed wet wiping cloths throughout facility stored on preparation tables and equipment. Instructed manager to store all wet wiping cloths in appropriate concentration of sanitizer in between use.

44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: observed plates, bowls, and pans on main prep line shelves to be stored with food contact surfaces exposed. Instructed manager to invert items or protect food contact surfaces from potential sources of contamination.

45. Single-use/single-service articles: properly stored & used
- comments: observed single service items and pizza boxes stored on the floor in the side dry storage location off main kitchen area. Instructed manager to elevate all items at least 6 inches off floor to allow for adequate cleaning and prevent potential sources of contamination.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed gasket of right side door of salad area preparation cooler to be torn. Instructed manager to repair or replace torn gasket.

57. All food employees have food handler training
- comments: observed no up to date food handler certificates for all employees on site at time of inspection. Instructed manager to ensure all food handling employees complete and maintain food handler training and maintain proof of completion. Ensure all new employees complete training within 30 days of hire.

58. Allergen training as required
- comments: observed no allergen training certificates for any managers at time of inspection. Instructed manager to obtain and maintain allergen training, as well as all other managers to maintain allergen training.
