- comments: 6-202. 15 observed front entrance/exit doors with a one-inch gap between the doors and a 1/2-inch gap under the doors. Instructed to make said doors tight fitting to prevent rodent entry.

- comments: 4-603. 15 observed no written procedure provided for large pots that are too large to be submerged in the 3-compartment sink/high-temperature dish machines. Instructed to provide.

- comments: 4-601. 11(c) observed accumulated debris on the lower shelves of the hot holding units that are located across from the cooking equipment in the prepared food prep area. Instructed to clean.

- comments: 5-501. 17 observed no covered receptacle inside the customer women's toilet room. Instructed to provide.

Instructed to eliminate pest activity, clean and sanitize affected areas, and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a). At the time of the re-inspection, observed about 75 live small flies on the wooden overhead, on the pictures and flying around the juicing food prep area. Instructed to eliminate live small flies activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority violation. Citation issued 7-42-090.
