- comments: observed cutting boards in center prep line on main floor covered in deep grooves and dark stains. Instructed to repair or replace and maintain smooth, cleanable cutting surfaces.

- comments: observed no hot water throughout facility. All sinks maintaining inadequate temperatures between 58. 7-65. 1f. Instructed to correct and maintain adequate hot water throughout facility. Priority violation 7-38-030(c). Citation issued.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered trash receptacle inside female employee toilet room located in basement. Instructed to provide.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed build up of grease on exterior of used cooking oil container located in garbage corral. Instructed to clean and maintain in good condition.

55. Physical facilities installed, maintained & clean
- comments: observed large crack in wall of basement hallway near manager's office and hole in ceiling of dry storage room in basement. Instructed to repair and maintain in good condition.

- comments: observed no allergen training certificates on site at time of inspection. Instructed all employees holding city of chicago foodservice manager certificates must complete allergen training and maintain records thereof on site at all times.

- comments: observed the following core violation not corrected from report #2453891 dated 10/27/2020: #55 'observed damaged floor in walk in cooler near dishwashing area. Instructed to repair and seal floor. ' instructed to correct and come into compliance. Priority foundation violation 7-42-090. No citation issued.
