- comments: observed black slimy substances on interior water components of the ice maker, dripping into ice "ice is used for human consumption". Instructed to turn off machine, remove ice and have interior of ice maker cleaned and sanitized and maintain. Priority foundation violation 7-38-005.

- comments: observed damaged weather strip at bottom of exit door #5. Instructed to make doors tight fitting to prevent pest entry and maintain.

- comments: water temperature observed at hand sink at far left side reaching 88. 7f in girls washroom located in the old wing building. Instructed to maintain proper water temperature of 100f. (two other handwashing sinks in said washroom were at proper temperature).

- comments: instructed to detail clean the kitchen exhaust hood filters to remove accumulation of excessive dust build-up and maintain.
