- comments: no written employee health policy on site for all employees. Must provide. Priority foundation violation 7-38-010 citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: no paper towels at two of the three kitchen hand washing sinks. Must provide at all times. Priority foundation violation 7-38-030(c) citation issued.
23. Proper date marking and disposition
- comments: observed tcs foods not properly date labeled. Cooked ribs pre-packaged for sale in a customer display cooler dated 9/6, whipped ricotta spread dated 9/3, other prepared foods such as dairy based strawberry shortcake with no expiration date in the same display case. Cooked pastas in the the cooks line cooler dated 8/26 assorted shredded cheeses with no dates, cooked rice dated 8/26, pasta sauce dated 8/26. Instructed all prepared, ready to eat tcs foods held longer than 24 hours must be properly date labeled with an expiration date no longer than 7 days. Priority foundation violation 7-38-005 citation issued.

- comments: all pre-packaged prepared foods in display coolers for sale must be properly labeled with the business name, address and list of all ingredients.

- comments: must install a splash guard between the dish hand washing sink and the clean stored pans/utensils on the right side of the washbowl.

- comments: broken/missing door handles on the kitchen two door tall reach-in cooler with exposed insulation. Must replace/repair to be smooth and cleanable. -----uneven, expanding spray foam used as a sealant between the wall and the utility service sink must be removed or made to be smooth and cleanable.

- comments: torn rubber door gasket on the kitchen two door tall reach-in cooler. Must replace.

- comments: back flow preventor on at the utility service sink where attached hoses were noted, is not located to be serviced and maintained. Must provide a visible back flow prevention device.

- comments: previous core violations from #2311981 on 9-10-19 not corrected. #51- prep area hand wash bowl pipe draining slowly (by rear exit/entrance door). Instructed manager said hand wash bowl must drain quickly. #53- no covered trash receptacle in washroom (ladies room only). Must provide and maintain. #58- no food allergen training certificates on premises. Instructed all city of chicago certified food managers must have food allergen training certificates on premises. Priority foundation violation 7-42-090 citation issued.
