- comments: observed no hand washing signs at hand washing sinks in prep, dish washing and bar areas. Instructed manager to provide hand washing signs at all hand washing sinks used by employees.

- comments: observed no consumer advisory and disclosure on brunch menu. Instructed manager to provide consumer advisory and disclosure on menu. Priority foundation 7-38-005

- comments: observed prep cooler not maintaining proper temperature 53. 1f instructed manager to repair and maintain cooler at 41f or below. Cooler is tagged held for inspection and must not be used until reinspected by chicago department of healthl. Priorty 7-38-005 citation issued

- comments: observed missing thermometers inside coolers. Instructed manager to provide thermometers in all coolers.

- comments: observed bulk containers not labeled in basement storage area. Instructed manager to label all bulk containers with common name of food.

- comments: observed over 100 mice droppings scattered on floor, on shelves under holding units, inside fryer cabinets in front prep area, on floors, shelves and bottom of prep tables in middle prep area, on floor and bottom of tables in dish washing area, on ledges and floor under shelving units, along walls and corners in basement prep and storage areas. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

- comments: observed food items not covered and stored on floor in prep, storage, basement and walk-in cooler areas. Instructed manager to cover and elevate all food items six inches off floor and maintain.

- comments: observed wiping cloths stored on prep table. Instructed manager to store wiping cloths in sanitizing solution between uses.

- comments: observed clean multi-use pots and pans stored on floor in dish washing area. Instructed manager to elevate all clean multi-use equipment off floor.
