- comments: observed bulk container with no labels. Instructed manager to label all containers with common name and maintain.
38. Insects, rodents, & animals not present
- comments: observed no updated log book with service reports or licensing information. Instructed manager to provide and maintain. Priority foundation 7-38-020 (c) citation issued.
55. Physical facilities installed, maintained & clean
- comments: observed clutter and unnecessary items stored on floor in rear near back door and in dry storage area. Instructed manager to remove unnecessary items and elevate necessary items 6 inches off floor and maintain.
