8. Hands clean & properly washed
- comments: observed food employee at register wipe his nose, touch beard/ mustache while and cough onto arms, proceed to process customer payment and lastly handle ready to eat foods (chicken, biscuits, sides) without washing hands during time of inspection. Instructed to properly wash hands and exposed portions of arms after said activities stated above, immediately before engaging in food preparation. Must maintain. Priority violation. #7-38-010. Citation issued.

38. Insects, rodents, & animals not present
- comments: observed approximately 60 small black live flies throughout rear prep area, on surfaces of hot water tank, cardboard boxes, unused cooking equioment stored on shelving units, ice machine all plumbing fixtures / sink basins in men and women toilet rooms. Recommended to contact pest control for service. Must eliminate fly acticity on premise and maintain. Priority foundation violation. #7-38-020( a). Citation issued.

51. Plumbing installed; proper backflow devices
- comments: observed hose connected to ice machine located in front near drive thru window leaking. Instructed to repair and maintain

55. Physical facilities installed, maintained & clean
- comments: observed broken wall base throughout walk in cooler throughout especially under wired shelving racks. Instructed to replace and maintain in order to prevent pest harboring.

60. Previous core violation corrected
- comments: observed previous core violations not corrected from report #2611603 on 2/3/25. They are as follows. . . 38)6-202. 15 noted a 1/4" gap between the front doors. Must provide a weather strip between the doors to prevent entry point for pests. 49)4-601. 11(c) must detail clean the interior of the walk in cooler: walls, floors, fan cover and shelving to remove spills and/or debris. Also, clean prep tables, storage shelving. 51)5-205. 15 observed a few plumbing issues at the 3 comp sink faucet, front hand sink underneath leaky pipe. Also noted standing water on the floor in the prep area. Must fine source and repair 55)6-501. 12 must detail clean the floors, walls and attacted piping behind cooking equipment. 58)2-102. 13 observed the city certified manager on duty without her allergen training certificate. Instructed to provied and maintain as required or citations will follow. Instructed to correct all previous core violations as required. Priority foundation violation. #7-42-090. Citation issued.
