- comments: no valid city of chicago food service manager on site while tcs foods are being served. A city of chicago food service manager shall be present at all times when tcs foods are being prepared and served. Priority foundation violation 7-38-012

- comments: observed the front doors in poor repair. Noted a gap between the exterior doors and a gap at the bottom of the interior doors. Must repair and maintain to prevent possible entry points for pests.

- comments: observed 30 mice droppings scattered on the floor and in the kids' bedding in the storage closet and noted 15 mice droppings scattered on the floor in the infant room storage closet. Must remove droppings and detail clean and sanitize all affected areas.

- comments: observed a leak underneath the hand sink in the women's washroom. Must repair and maintain.

- comments: garbage receptacle with lid needed inside women's toilet room. Instructed to provide.

- comments: observed gaps along the wall base and floor in the closet where droppings were found. Must seal all holes and openings wherever needed to prevent further pest issues.

- comments: must detail clean walls to remove all cobb webbing and floors in the kitchen along the wall base and in corners and where droppings were noted. Also clean the light shields to remove dead bugs wherever needed.
