- comments: observed the food handler at the front servinig counter not wearing any type of hair restraint. Instructed manager that all food handlers in the food prep and serving area must wear and maintain some type of hair restraint.

- comments: observed dust and debris on the fan cover inside the walk in cooler. Must detail clean and maintain.
55. Physical facilities installed, maintained & clean
- comments: must detail clean the floors along the wall base underneath the cooking equipment, hand sink and 3 comp sink to remove debris, spills and/or grease. Also detail clean the air vents in the front seriving area to remove dust.
