Report date: Dec 15, 20252. City of chicago food service sanitation certificate
- comments: observed nocity of chicago food service sanitation certificate for employees working on site. Informed a certified manager with a current city of chicago certificate must be on premises at all times and a valid must remain posted on site whenever time and temperature control for safety(tcs) foods are being prepared, handled and/or served. Priority foundation violation #7-38-012. Citation issued.
6. Proper eating, tasting, drinking, or tobacco use
- comments: observed personal food item stored inside walk-in cooler. Instructed manager to remove. Employees shall eat and drink only in designated areas where the contamination of exposed food cannot result.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed at hand soap and drying provisions above 3 compartment sink instructed to remove.
22. Proper cold holding temperatures
- comments: noted internal temperatures of the following time/temperature control for safety food items reach-in cooler and in prep area to be improper: eggs at 64. 3f in prep area under toaster, cut melon at 50. 4f pic immediately and voluntarily disposed of 10 lbs of products worth $175. 00 through denaturing process. Priority violation #7-38-005. Citation issued.
40. Personal cleanliness
- comments: observed food handlers prepping food without hair restraints. Instructed person in charge that all food handlers must wear a hair restraint.
43. In-use utensils: properly stored
- comments: observed cup for dry storage containers being stored directly inside of the container. Instructed to properly store all utensils between use.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed 3-compartment sink stoppers missing/not working at left side and middle compartments. Instructed manager must provide working stoppers at 3-compartment sink.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed the toilet room in need of cleaning. Instructed to detail clean and maintain at all times.
55. Physical facilities installed, maintained & clean
- comments: observed food debris and spills on the floors and surfaces throughout the facility. Instructed to detail clean under all shelving units, interior of prep coolers, hot-holding units, prep tables and deep fryers to remove spills, food debris, dirt buildup and/or grease. Must maintain at all times.
58. Allergen training as required
- comments: observed the city of chicago food certified manager with no proof of allergen training certificate. Instructed all city of chicago certified food managers obtain this certificate.
