39. Contamination prevented during food preparation, storage & display
- comments: observed employee pizza box with partially eaten pizza stored in steamer directly on top of hotdog buns. Instructed manager to ensure employee food is kept seperate from consumer food and is stored in a way that does not cause cross contamination. Manager discarded pizza box. Priority foundation violation 7-38-005 citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: observed box of pizza stored directly on floor. Instructed manager to ensure food is not stored on floor.

49. Non-food/food contact surfaces clean
- comments: observed heavy soil and food debris build up on pizza oven. Instructed manager to clean and maintain.

52. Sewage & waste water properly disposed
- comments: observed waste water from the grease trap seeping to the floor. Rear area flooded with water from grease trap. Water was spreading into the front prep/cooking area. Cardboard boxes on floor around greasetrap to absorb water. Instructed manager to service grease trap. Priority violation 7-38-030(c) citation issued. Employees ceased food prep and began to clean accumulated water. A new grease trap was brought to the site to replace the existing unit.

55. Physical facilities installed, maintained & clean
- comments: observed floors under and around all equipment and around ice machine and c02 tanks with accumulated food debris and litter. Instructed manager to deep clean floors throughout facility and maintain clean.

56. Adequate ventilation & lighting; designated areas used
- comments: observed grease and debris build up on walls, lights, and filters above griddle. Instructed manager to clean and maintain.
