- comments: observed no consumer advisory disclosure on the menu for raw or undercooked foods. Management instructed to disclose or describe which foods can be ordered raw or undercooked and link those items to the reminder statement on the menu must add the following sentence before the reminder statement: "these items can be ordered raw or undercooked or may contain raw or undercooked ingredients. - comments: clean the top of the pizza ovens.

- comments: observed the drain line for the exposed hand washing sink behind the main bar leaking. Management instructed to repair. Observed a slow drain at the exposed hand-washing sink behind the small bar. Management instructed to repair so that the sink drains properly.
