- comments: observed no employee holding valid city of chicago foodservice manager certificate on site while tcs foods were being prepared. Instructed to maintain a certified manager on site at all times while open and operating. Priority foundation violation 7-38-012. Citation issued.

- comments: observed no soap at rear hand washing sink located next to 3 compartment sink. Instructed to maintain soap at hand sink at all times. Priority foundation violation 7-38-030(c). Citation issued.
