- comments: observed no employee health policy provided on site. Instructed to provide verification for each employee on site at all times. Priority foundation. 7-38-010.

5. Procedures for responding to vomiting and diarrheal events
- comments: observed supplies missing for vomit and diarrhea clean up kit (apron, liquid spill absorbent, disposable scrapper/shovel, spray bottles, disinfectant against norovirus. Must provide all necessary supplies as outlined in policy. Priority foundation. 7-38-005.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed improper water temperature at hand washing sink at 84. 9f at bar area. Proper water temperature must be at a minimum of 100. 0f or higher through a mixing valve or combination faucet. Priority violation. 7-38-030(c). Citation issued.

- comments: observed dirty cutting board in prep area with dark stains and deep grooves. Must clean and maintain.

- comments: observed water temperature at 3-compartment sink at 84. 4f at bar area. Water temperature must be maintained at 110. 0f or higher at all times. Priority violation. 7-38-025. Citation issued.

- comments: observed heavy grease build up inside of cooking fryer. Must clean and maintain.

- comments: observed ventilation not working in men's bathroom. Must repair issue.

- comments: observed no food handler training certificate on site. Must provide for all current employees.
