- comments: observed approximately 10 rodent droppings in back of hot water heater. Remove all evidence of rodents and clean and sanitize all affected areas. Priority foundation violation 7-38-020 (a)

- comments: observed no pest control log book on site. Must provide pest control log book with a licensed pest control service and all required information. Priority foundation violation 7-38-020 (c)

- comments: observed ice scoop stored in container hanging inside handsink. Do not store ice scoop in hand sink. Store in a clean dry container on top of machine away from possible contamination.

- comments: observed 2 outside grease storage units not mainained with an accumulation of grease on the top, outsides, and around the containers. Observed container not properly covered and the accumulation of grease not minimized to deter insect and rodent attraction. Priority foundation violation 7-38-020 (b)

- comments: observed ceiling above food preparation area with missing broken ceiling tiles. Replace and maintain all ceilings within facility.

- comments: observed violations not corrected from report # 2590383 dated 3/1/24 including the following violations: #55 observed a heavy accumulation of dust, grease, food splatter and food debris on the walls and floors especially along the hot food service line under and around the cooking equipment and non cooking equipment. Instructed to clean and maintain all areas. #47 observed accumulated grease and food debris on interior and exterior surfaces of cooking equipment, prep tables, coolers, freezers, shelving units. Instructed manager to clean and maintain. #49 observed the inside of the deep fryer oil filtering unit with a build up of grease and food debris.
