2. City of chicago food service sanitation certificate
- comments: 02 found no certified food manager on site while tcs foods (ham, dairy,cold sandwiches etc) are being prepared and served. Instructed person in charge, a certified food manager with city of chicago foodservice sanitation certificate must be present at all times when handling tcs foods. Priority foundation violation 7-38-012 citation issued
5. Procedures for responding to vomiting and diarrheal events
- comments: found no procedure/plan for responding to vomiting and diarrheal events. Instructed to provide and maintain required procedure/plan and appropriate supplies or kit. (necessary items at a minimum: gloves, face masks, disposable mops and appropriate sanitizer/spray that kills norovirus) priority foundation violation 7-38-005 citation issued

- comments: observed no hand soap at the exposed hand washing sink in front prep area and in washrooom. Management instructed to provide soap at all hand washing sinks at all times. ( corrected on site - employee provide soap for handsink infront prep area and in washroom) priority foundation violation 7-38-030(c) citation issued.

- comments: instructed to provide thermometer visible inside prep cooler, reach-in coolers and in walk-in cooler.

- comments: found no label for pre packaged rice pudding, pineapple gelatine, beet juice, carrot juice, orange juice etc displayed for sale. Instructed manager, all prepackaged foods on premises for sale must have proper identification. (name of product, ingredients, net quantity of contents, name and place of business of manufacturer, packer or distributor).

- comments: instructed to detail clean and maintain interior and exterior surfaces of prep cooler in front prep area, interior surfaces of display cooler and reach-in coolers in rear prep area. Instructed to detail clean and maintain interior surfaces of handsink cabinet inside washroom.

- comments: instructed to transfer food products from an open original can container pineapple, jalapeno peppers) into food grade container for proper food storage inside reach-in cooler in rear prep area.

- comments: found large mixing bowls in rear prep/baking area unable to submerged in a 3 compartment sink. Instructed to provide a written detailed washing, rinsing and sanitizing procedure for food-contact equipment that is too large to be submerged in the three compartment sink for proper wash, rinse and sanitize.

- comments: observed drainline from walk-in cooler draining into one compartment of the 3 compartment sink. Instructed to properly drain onto floor drain. Instructed to provide 3 working drain stoppers for 3 compartment sink in rear dishwashing area.

- comments: instructed to detail clean and maintain interior and exterior surfaces of all baking equipment (mixers, dough maker etc)

- comments: instructed to repair slow draining 3 compartment sink in rear dishwashing area. Instructed to repair loose faucet at handsink inside the washroom.
