- comments: observed no certified food manager on duty nor certificate posted to view while tcs foods are being prepared,handled and served such as (fried chicken,fish,cole slaw,french fries,etc)must be on site and provide a certificate. (priority foundation 7-38-012)(citation issued)

- comments: observed no clean-up policy procedure and items for vomiting and diarrhea must provide and maintain. (priority foundation 7-38-005)(citation issued)

- comments: must label food shakers/food storage containers/bins when food is not in original package.

- comments: observed evidence of appx. 40 or more mice droppings on floor in walk in cooler next to shelving units, on floor behind large on wheel food storage containers,on a baking pan on lower shelf with chemical stored on it. Must remove droppings clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 2/21/24. (priority foundation 7-38-020(a)(citation issued)

- comments: observed a gap along bottom of both front entry doors and rear door must make doors tight fitting to prevent rodent entry.
