- comments: observed person in charge without proof of food safety management training during time of inspection. Must provide.
2. City of chicago food service sanitation certificate
- comments: observed no valid city of chicago food service sanitation certificate posted on site during time of inspection. Must provide. Informed person in charge, the original city of chicago food service certificate must remain posted on site at all times. Priority foundation violation #7-38-012. Citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed the following sinks not maintained with hot water for proper handwashing: womens washroom 61. 5 f, mens washroom hot water 73. 6 f. Instructed person in charge hot water at all hand washing sinks must be at least 100f. Water temperature was adjusted to 104. 5 f during inspection priority violation #7-38-030(c). Citation issued.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed ice machine located in basement with a heavy build-up of black substance dripping onto ice cubes in below ice bin from the top interior and on the interior plastic sheild. Stored ice is used for human consumption. Instructed to remove ice, deep clean,sanitize and maintain. Priority foundation violation #7-38-005. Citation issued
41. Wiping cloths: properly used & stored
- comments: observed several green wiping cloths on bar surface. Instructed to properly store wiping cloths in a sanitizing solution between uses to prevent contamination of equipment & food.
55. Physical facilities installed, maintained & clean
- comments: observed clutter throughout basement. Noted items stored on floor aling the walls. Instructed manager to remove all unused articles and equipment to prevent pest harborage. All kept items are to be stored on proper shelving that is non-porous, smooth and easy to clean
57. All food employees have food handler training
- comments: observed no proof of training for all food handlers. Instructed to provide and maintain records at all times.
