- comments: 6-501. 12: **c** cleaning, frequency and restrictions must clean debris build up from floor & drains under, around,along wall bases in dry food & paper storage area.

- comments: observed previous priority foundation violations not corrected within specified time frame on the inspection report 2632128 dated for 3/4/26: (38)6-501. 111: **pf** controlling pests (pest control log book) observed pest control logbook with reports from 07/2025 service report. All retail food establishments must maintain a logbook which shall be kept for a period of twelve (12) months and include a copy of the service tickets and the pest control company's illinois department of public (idph) health license number. Instructed to provide. Instructed person in charge that the violation shall now classified as a priority item due to previous noncompliance. Must correct before re-inspection. Priority violation 7-42-090. Citation issued.
