- comments: observed no one on site with a city of chicago certified food manager certificate while serving time/temperature controlled foods including eggs. Priority foundation violation 7-38-012

10. Adequate handwashing sinks properly supplied and accessible
- comments: a sign or poster that notifies food employees to wash their hands is not provided at handwashing sink in back prep-area and all gender restroom used by food employees. Provide a clearly visible handwashing sign at each hand sink.

39. Contamination prevented during food preparation, storage & display
- comments: observed standing mixer plugged in underneath sewer line and unprotected ceiling. Remove mixer from basement immediately. No open food or food preparation is allowed in basement. Priority foundation violation 7-38-005

- comments: all gender restroom lacks a covered receptacle for disposal of feminine hygiene products. Provide at least one covered trash receptacle in restroom.

- comments: lighting in the mop closet is found to be insufficient. Light intensity shall be sufficient so that the area, all utensils, equipment, and food items are easily visable.

- comments: observed previous priority foundation violation from report #2569013 dated 12/28/22 not corrected including previous core violations #55 ceiling must be smooth and easily cleanable in basement area over dry storage area #56 broken protective shielding of light fixture in prep area. Replace. #57 no food handler certificates could be provided # no allergen training certificates on site. Priority violation 7-42-090
