- comments: must properly label food storage containers when food is not in the original container.

- comments: observed approximately 40 live small flies on ceiling and walls in front prep area and rear utility mop sink area. Instructed management to clean and sanitize affected areas. Additional pest control service is needed to eliminate flying insect problem priority foundation violation 7-38-020(a) citation issued.

- comments: outer openings not protected. Observed 1/2 inch opening on bottom of revolving entrance door. Must make door rodent proof to prevent pest entry.

- comments: observed wiping cloths stored on top of prep tables in front and rear prep areas. Must properly store wiping cloths in a sanitizing solution when not in use.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered waste receptacle provided in the unisex employee washroom. Must provide covered waste receptacle and maintain.

- comments: observed dirt and debris on floor under and behind refrigeration units in front prep and rear prep area/dish area. Must clean and maintain floor clean at all times.
