- comments: observed ice machine located in rear prep area with yellow slime and mold-like buildup on internal panel of equipment dripping onto ice used for human consumption. Instructed to clean and sanitize food contact surface and maintain. Priority foundation violation. 7-38-005. Citation issued.
38. Insects, rodents, & animals not present
- comments: observed approximately 35 small and large live flies flying throughout the following areas during the time of inspection. . . Rear prep/dishwashing area near piping and drains, on equipment surfaces of serving line, on shelving units in dry storage room and indoor refuse area. Instructed to contact pest control for additional service. Must eliminate pest activity. Priority foundation violation. #7-38-020 (a). Citation issued.

- comments: observed approximately 1/4 inch gap between door and floor in compactor room. Instructed to enclose gap to prevent pest entry.

- comments: observed piping attached to 3 compartment sink in rear prep dripping into bucket, noted faucet in women's toilet/locker room washbowl on right handside hot water handle leaking as well as sink on left-hand side not draning properly with water pooled inside sink basin. Instructed to repair and maintain in good condition.

- comments: instructed to provide garbage can in women's toilet room with covering.

- comments: observed no wall base and yellow wall in disrepair leading into prep. Instructed to repair wall and provide base.
