- comments: observed ice machines locted in front serving station area not maintained. Observed interior of ice machines behind panel with excessive amount of black and brown mold like substance. Ice is being used for human consumption. Instructed to disposed of the ice and clean and sanitize ice machines. Priority foundation violation 7-38-005 citation issued.

- comments: bakflow prvention device not locate at the ice machines located in front serving station area. Instructed person in charge must provide/make a visible backflow prevention device for ice machines.

- comments: observed heavy dust build up on ceiling ventilation vent above utility mop sink. Must clean and maintain.
