- comments: observed black slimy substances on interior water components of the ice maker, dripping into ice "ice is used for human consumption". Instructed management to turn off machine, remove ice and have interior of ice maker cleaned and sanitized and maintained. Priority foundation violation 7-38-005, citation issued.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed torn door gaskets inside of the 4 drawer cooler underneath the cooking grill. Instructed to replace the torn door gaskets and maintain.

49. Non-food/food contact surfaces clean
- comments: instructed to clean the following equipments and maintain: prep tables (tops and bottoms), exterior surfaces of fryers and other cooking equipments, under grill and shelves inside of refrigeration units.

51. Plumbing installed; proper backflow devices
- comments: observed water leak at base of faucet at the dishwashing 3-compartment sink. Instructed to repair plumbing leak and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed food debris and spills on the floors and surfaces throughout the prep/dishwashing area/dry storage room. Instructed to detail clean under all shelving units, around dish machine, prep tables and all cooking equipment to remove spills, food debris, and dirt buildup. Instructed to replace waterstained ceiling tiles inside the mens washroom and maintain.

56. Adequate ventilation & lighting; designated areas used
- comments: instructed to detail clean ceiling ventilation covers in prep/dishwashig area to remove accumulation of dusts and maintain.

- comments: observed burnt-out light bulbs for light fixtures at rear dishwashing 3-compartment sink area. Instructed to replace burnt-out light bulbs and maintain adequate lighting at all times.

- comments: observed no food handler certificates provided on the premises. Must show proof of training for all food handling employees.

- comments: observed the certified food manager with expired allergen training certificate. Must provide and maintain.
