1. Person in charge present, demonstrates knowledge, and performs duties
- comments: observed no person in charge with valid chicago food sanitation certificate.

2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty while tcs foods are being prepared, handled, and served such as (chicken, pork, lamb, etc. ) must be on site at all times. Priority foundation 7-38-012. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed handwashing sink in prep area not stocked with hand soap. Informed person in charge that hand soap must be available at handsinks at all times. Food handlers obtained hand soap for the hand sink during the time of inspection. Priority foundation violation #7-38-030(c). Citation issued.

- comments: observed missing handwash signage at bar handwash sink. Instructed to provide and maintain.

- comments: observed the kitchen prep sink not maintained with hot water: improper temperature of hot water observed at (63f). Instructed person in charge of hot water at all hand washing sinks must be at least 100f. Priority violation #7-38-030(c). Citation issued.

16. Food-contact surfaces: cleaned & sanitized
- comments: noted low-temperature dish washing machine in use not properly sanitizing. The sanitizing strength of the sanitizer (chlorine) at the time of inspection was 0 ppm. Equipment must be properly sanitized at a minimum of 50 ppm. Instructed not to use unit until reinspected by chicago department of health (cdph). Tagged unit. Priority violation #7-38-025. Citation issued.

22. Proper cold holding temperatures
- comments: observed tcs food held at improper internal temperature in the rear prep area, such as 5 lbs of chicken at 47. 2f. The manager discarded and denatured a total of 5 lbs at all 50 cold tcs. Food must maintain 41f or below. Priority violation. Citation issued 7-38-005.

40. Personal cleanliness
- comments: observed a food handling employee not wearing a hair restraint while preparing food. Instructed management that all food handling employees must wear a hair restraint while preparing food.
