- comments: observed ice scoop laying on top of ice in bar. Instructed manager to store ice scoop handle up above ice to prevent contamination.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: found the three compartment sink not maintained; sink is leaking when water from compartments are drained; water is been collected with containers. Instructed management to get drainage system serviced. Priority foundation 7-38-025. Citation issued

55. Physical facilities installed, maintained & clean
- comments: observed debris on floors along walls and corners in prep, utility and storage areas. Instructed manager to clean and maintain.

- comments: observed no food handlers certificates on site. Instructed manager to comply with the idph food handler requirement.
