- comments: observed no clean-up policy procedure and items for vomiting and diarrhea. Must provide and maintain. (priority foundation 7-38-005)

- comments: observed improper temperatures of tcs cooked and ready to eat foods such as 5lbs. Cooked steak at 91. 4f and 4lbs. White turkey at 46. 8f. Inside of 3- door reach in cooler. Product was discarded by manager. Must have cold holding foods at 41. 0f or below. Appx. 9lbs. $75. (cos)(citation issued(priority 7-38-005)

- comments: observed evidence of appx. 25 or more mice droppings inside of fryer cabinet,on floor next to bait box at grease trap and on floor in mopsink closet. Must remove droppings clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 4/21/25. (priority foundation 7-38-020(a)(citation issued)

- comments: observed a gap along bottom of front door,worn sweep on rear door must make doors tight fitting to prevent insect entry.
