- comments: observed no food employee with city of chicago certified manager training or original certificate on-site while tcs foods are handled and prepared (milk tea, ice cream). Instructed to ensure at least one food employee with certified manager training present during all hours of food operations and maintain original certifcate on-site at all times. - comments: no probe thermometer or other food temperature measuring device present on-site for ensuring attainment and maintenance of food temperatures. Instructed to provide food temperature measuring device such as a probe thermometer for measuring food temperatures, and maintain calibrated and available for use on-site at all times. - comments: observed plastic single-use cup used as a scoop in dry food storage container in the rear area. Instructed to remove single-use cup and replace with multi-use utensil with a handle to prevent contamination while scooping and maintain.

- comments: observed nonfood-contact surfaces (interior cavity/interior door surfaces) of stainless steel cabinet in front prep area to be soiled with potable water and rust debris. Instructed to clean nonfood-contact surfaces of equipment at adequate frequency to prevent build up of soil and maintain.

- comments: observed no drain stoppers present on-site for use during warewashing at the 3-compartment sink. Instructed to provide at least one drain stopper in good repair for each basin of the 3-compartment sink and maintain on-site at all times for use in warewashing.

- comments: observed floor of walk-in cooler soiled with condensate water and food debris. Instructed to clean floors of walk-in cooler and other physical facilities at adequate frequency to prevent build up of soil and maintain.
