2. City of chicago food service sanitation certificate
- comments: observed no manager with a valid/physical city of chicago food certificate on site at time of inspection. No manager present for coffee shop area located in first floor lobby while open and operating. Instructed current person in charge that a manager with a valid city of chicago food certificate must be present during all hours of operation. Manager with certificate arrived on site at 11am during the inspection. Priority foundation violation, #7-38-012. Citation issued.

38. Insects, rodents, & animals not present
- comments: observed about 15 small dead flies present on the shelf in the white standing galaxy cooler in the basement prep area. Instructed to remove all dead insects, and to clean and sanitize area.

44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: observed equipment, pots, pans, and various items stored on the floor and below shelves in the equipment storage room in the basement/lower level area in back hall. Instructed to elevate all items at least 6 inches to allow for appropriate cleaning.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no hot water available at the 3 compartment sink at minimum temperature of 110f at time of inspection. Turned hot water handle and no water available. Cold water was available at time of inspection. Employee stated a maintenance ticket had been opened for the hot water, and the maintenance employee stated that he was unaware of any issues with 3 compartment sink hot water. Maintenance employee inspected and turned shut off valve below 3 compartment sink to the "on" position and restored water to adequate temperature and pressure at time of inspection, 111. 2f. Hot water was available throughout facility and hand sinks at time of inspection. Instructed to ensure hot water availabitily at minimum of 110f in 3 compartment sink at all times to maintain warewashing equipment. Priority violation, 7-38-025. Citation issued.

49. Non-food/food contact surfaces clean
- comments: observed interior shelves and bottom shelf of white standing cooler in basement prep are to be heavily soiled with food debris and spillage. Observed interior bottom shelf of right side oven in the jparker kitchen located behind the bar in the rooftop. Instructed to detail clean and maintain free of excessive food debris and spillage build up.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no trash can with lid in the employee bathrooms (one unisex and one women's) in the basement prep area in the hall. Instructed to provide a covered trash in each bathroom.

55. Physical facilities installed, maintained & clean
- comments: observed heavy build up of debris and spillage on the floor surrounding the ice machine in the rooftop/jparker bar area. Instructed to detail clean and maintain area free of excessive food debris and spillage.
