- comments: observed no certified food manager on duty while tcs foods are being prepared, handled, and served, such as (italian beef, pasta sauce, chicken & fish)must be on site at all times. Manager arrived on site at 12:40 p. M. (priority foundation 7-38-012)(citation issued)(cos)

- comments: observed several fly strips hanging from ceilings in prep and dishwashing areas must be removed.

- comments: must provide a thermometer for sliding glass reach-in cooler.

- comments: must label food storage containers (bulk bins, squeeze bottles, shakers) when food is not in original package.

- comments: observed an infestation of appx. 50 or more live roaches crawling in between and on top of unnecessary articles at the front service counter and the motor of the glass bread cooler. Also appx. 100 or more live small flies crawling in floor drains in the washing area, on walls, ceilings, food storage containers, dishes, garbage cans, etc. , throughout the premises, including the toilet room. Must have a pest control operator service premises on or after 10/18/24. (priority 7-38-020(a)(citation issued)

- comments: must store wiping cloths in a sanitizing solution when not in use on prep tables.

- comments: must clean excessive grease and food debris build up from pipes of cooking equipment, prep tables, all reach-and & prep coolers, grill table, legs of cooking equipment, and tables. Defrost ice build up from glass chest freezer.

- comments: must repair or replace leaky water line connection at 3-compartment sink.

- comments: observed outside grease container encrusted with grease and food debris build up and food boxes thrown on ground around grease container. Must remove grease and food boxes from the ground and clean and maintain the area. (priority foundation 7-38-020(b)(citation issued)
