- comments: observed no hand washing signs or posters near hand washing sinks in food preparation areas and washrooms. Must provide hand washing signs or posters at all hand washing sinks and maintain.

- comments: must properly label food storage containers when food is not in the original container.
38. Insects, rodents, & animals not present
- comments: outer openings not protected. Observed front entrance door not rodent proof door has 1/2 inch gap underneath door. Must make door rodent proof to prevent pest entry.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed rusty food storage shelving in basement food preparation area. Must repaint or replace shelving.

- comments: observed excessive amount of ice build up inside chest freezer located in the rear of basement. Must remove ice and clean freezer.

- comments: observed exposed hand washing sink draining slow in first floor kitchen prep area. Must repair and maintain.

- comments: observed excess of dust build up on ceiling ventilation vents in washrooms. Must clean and maintain. Observed peeling paint on wall on rear north side of basement. Must repaint and maintain. Observed water damaged ceiling tiles above dry food storage shelving in the basement. Must repair or replace.
