38. Insects, rodents, & animals not present
- comments: observed approximaltely 200 or more small rodent droppings in the following areas during time of reinspection : in front windowsill of window located front of premise, under front hot holding unit on the floor, under cooking equipment (grill, steamtable), on bottom area of prep tables, under 3 compartment sinks, in closet near rear exit, alongside cooling units under filing cabinet in kitchen area, under safe, on the floor around and behind kitchen equipment, in the storage room in rear of kitchen on the floor in all corners , in the furnace room on the floor, inside of both the walk in coolers on the floor and on top nof box containing plaintains, in the office area on the floor and desk. Instructed to clean all areas and have pest control company service establishment. Priority violation. #7-38-020 (a).

57. All food employees have food handler training
- comments: 2-102. 13 instructed that all food handlers must obtain the required food handler training.

60. Previous core violation corrected
- comments: previous core violations not corrected from june 5, 2025 as required; must correct and maintain. 49 observed food debris on interior and exterior surfaces of cooking equipment, prep tables, coolers, freezers, shelving units, holding units, bulk containers. Instructed manager to clean and maintain. 55 instructed to clean floors, walls and ceiling vents in kitchen, storage and other areas to remove dirt and dust. Clean light shields where needed and vent fan covers in washrooms to remove dust build up. 58 observed an expired food allergen training certificate. Instructed that both city of chicago food service managers must obtain the required food allergen training as instructed.
