Aug 4, 2020 55-6-501. 11: **c** maintained in good repair, facilities (facilities not in good repair) water stained ceiling tiles in the dining area close to the window and above the hand sink in the front area. Must replace the ceiling tiles and maintain. Aug 4, 2020 56-4-202. 18: **c** ventilation hood systems, filters ventilation hood/filter with grease and dust build-up. Must detail clean and maintain. Aug 4, 2020 #58-allergen training is required for all employees that have a city of chicago food sanitation certificate. Instructed to complete training and maintain certificate on site. Instructed to correct noted violation and maintain by 1/22/21. Priority foundation violation 7-42-090, no citation issued.

I started to have an extremely stomachache accompanied by vomiting. The first ones were very painful, I vomited 7 times during the night accompanied by uncontrollable diarrhea.
I could not sleep all night, the diarrhea and vomiting continued into the morning.
I didn't eat all day because I was very sick.
I waited until the night of Friday to Saturday with a lot of concern, I was right because I also vomited 6 times and slept very little.
On Sunday morning it is a little better after more than 2 days of fasting.

I started experiencing intestinal cramps. Then, when we got back to the hotel, the nausea started. That's when my spouse told me she had been feeling ill as well. She was the first to vomit and I followed there after. We struggled to finish packing for the morning flight. We only had fluids in the form of water and apple juice for the entire journey. She only experienced nausea, intestinal cramps and vomiting. I on the other hand had all the symptoms including chills, nausea, intestinal cramps, vomiting, fever and diarrhea.
