Report #2591630 dated march 26 2024 instructed facility to #51 ensure all plumbing fixtures are in proper working order and to #55 declutter basement. Observed toilet in downstairs men's single service toilet room unable to flush. Observed heavy clutter in basement are of facility. Instructed manager to address all previous core violations. Priority foundation violation 7-42-090 citation issued.
