- comments: observed no hand washing sign at hand washing sink in staff washroom. Instructed manager to provide hand washing signs at all hand washing sinks used by employees.

- comments: observed over 100 mice droppings scattered on floor along walls and rolling cart in kitchen, on floor along walls and corners in storage room, on floor along walls and corners in utility closet, on floor along walls and corners in staff washroom, on floor, computer tables and window ledges in classroom and office areas. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

- comments: observed single service utensils not inverted in prep area. Instructed manager to invert all single service utensils to protect lip contact surfaces from contamintion.

- comments: observed plumbers putty around drains inside 3-compartment sink. Instructed manager to remove all plumbers putty around drains and maintain.

- comments: observed openings in ceiling in kitchen, openings around pipe fittins in storage room and under sinks washroom. Instructed manager to seal all openings in walls and ceilings in all areas.
