- comments: observed black and yellow/green substance on interior ice chute. Ice is used for human consumption. Instructed to remove all ice, deep clean, sanitize and maintain. Priority foundation violation #7-38-005. Citation issued

- comments: observed a footnote reminder statement on bottom of the menu but missing disclosure by astericks for related menu items (eggs and wake'n bacon burger) and no disclosure or reminder on online menu for (eggs - 'cooked to your style' & wake'n bacon burger). Priority foundation violation #7-38-005. Citation issued

- comments: noted dead large and small roaches in lobby heater closet and in various locations on the floor in upstairs dry storage area. Instructed to remove to properly monitor new activity.

- comments: noted heavy dust on ceiling outlet above waffle iron on prep table, a build up of dust on vents in kitchen above prep table and dust on vents in washrooms. Instructed to clean and maintain.

- comments: observed no proof of allergen training for all certified food service managers. Instructed any persons with the city of chicago food sanitation certificate to comply with completion of training and provide and maintain documentation on site at all times.
