- comments: observed no exposed sink for handwashing located in first floor dishwashing area. Instructed to install exposed handwashing sink in first floor dishwashing area for convenient use by employees. Priority foundation violation. 7-38-030(c) no citation issued.

- comments: observed no asterisk on menu identifying animal derived foods that are served undercooked or cooked to order. Observed no disclosure on menu stating that asterisked animal derived foods are served undercooked or cooked to order. Instructed to add asterisk identifying all animal derived foods that are served undercooked or cooked to order and to add disclosure to menu stating that animal derived foods with an asterisk are served undercooked or cooked to order. - comments: observed no splash guards on handwashing sinks in bars on second floor. Observed no left side splash guard on southside handwashing sink in eastside bar and rightside splash guard on southside handwashing sink in eastside bar. Instructed to install splash guards on said sinks in bars on second floor to prevent contamination of equipment.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no irreversible temperature indicator for measuring surface temperature of utensils and other dishes. Instructed manager to provide. Priority foundation violation 7-38-005 no citation issued.

51. Plumbing installed; proper backflow devices
- comments: observed toilet out of order in men's washroom second floor(left stall on westside). Instructed to repair toilet in said stall of men's washroom on second floor. Observed drain line leading from ceiling into handwashing sink in first floor prep area near smoker. Instructed manager to relocate drain line from ceiling to floor drain next to handwashing sink in prep area near smoker in first floor prep area. .

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacle in all gender employee washroom and employee women's washroom on first floor and southside stall(on north wall on rightside) in women's washroom on second floor. Instructed to provide covered receptacles for sanitary napkins.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed damaged dumpster lid on first dumpster and observed missing dumpster lid on second dumpster. Observed dumpster lid pushed upwards creating an opening, lid is not tight fitting on first dumpster. Observed no dumpster lid on second dumpster. Instructed to replace damaged and missing dumpster lids and maintain tight fitting lids on dumpsters. Priority foundation violation 7-38-020(b) citation issued
